Pass the guilt and stay strong by your mental health, screw them. Quit and start breathing again. I talk from the experience.
There are many jobs but if you end burning yourself all of them will taste like shit. quit before it eats you alive more.
I really hope you get better. -

Do you have any savigns? (you don't need to answer this at all) Could you just drop the job in like "fuck you all and good riddance"?
I know there are bills to pay always, but there is a moment when you need to choose you over, well, hell itself. It is okay to quit.
In my case telling them "This is my 15 days notice" just did a lot. and from there I was able to focus again and breathe.
Setting boundaries helps too: now work on weekends and like hell I am gonna do extra hours, they don't pay enough for us to suffer their shortcomings.
I will assume that you work in america so absence due to mental health is not an option? -

It is okay to quit.
I did it on a shitty place long time ago. Best decision mental health wise that I ever made.
Since the day I created here an account I only see you in quite a personal hell.
I hope its gets better for you somehow. -

I understand the presure can make one have shity behaviour, but don't pay it on others.
That never will change if you or your colleages allow it. I do not say that the PM is a bad person, but he/she should do better. -

On a previous company, a poor junior dev did not understand how the code was failing when executed, but all the test ok.
Turns out the dev placed each case on try/catch blocks.
The good news is that the Junior Dev understood what was wrong after a bit of a demonstration and removed the tey/catch blocks.
The very bad news was a 28% of the test showed as Ok...
So I can understand the frustration fixing this kind of stuff. -
I hope you find work soon.
Btw, did you talk to HR (like serious talk about the shit enviroment they had) ? Or just say that you will leave the company? -
