I've personally experienced this in two scenarios: being in flow, and when there's some sort of ongoing production-related crisis. Surprisingly easy to forget to eat in either case.

Git for version control (previously Subversion), wiki platform of choice (Confluence in our case) for docs.
That's at work. For personal projects I use Git in conjunction with Github for light documentation. -
Being new actually puts you in a pretty good position to point out opportunities for improvement. You can probably mention it as soon as you want to.

Or the daily 30 minute "check in" meetings with the PM during crunch time which inevitably run over every single time.

Reminds me of the time a tester came upon our internal API documentation page and thought they found some sort of exposed master control center and reported it as a critical security bug.
