I started on my commodore 128D by typing over code from magazines. After that I started modifying them a little. Then I started making programs that drew little sketches on the screen using vb lineto commands. All this when I was 10years old.

Almost any internally tool developed by a non-dev who has read a book 'learning to program in 21 days' and now thinks he can code. Usually it is developer in excel and as years pass complete departments depend on it until the moment a consultant is hired to completely rewrite it without any specs.

Why do people keep inventing new object serialisations when there is already JSON and XML? Why do people think that a custom format and parser would be better than an industry standard and a parser that has years of development improvements? :-S

I once inherited a project that had been outsourced for more than 6 months to a company at the other end of the world. Although the PM had almost daily contact with the developer, the project wasn't technically followed up.
I had already recommended code reviews 3 months before I inherited the project. But of course these had never happened.
The project contained all the nice-to-have features, but the core wasn't working. Loading the home page (with 20 records from a DB) took 15 minutes.
We then had roughly a month the get the project straight.4 -
